I'm having trouble creating a mega evolution for my pokemon

I created an item, gave it the same properties as the other mega stones, created a mega form for my pokemon (form 30), gave it to my pokemon and gave myself a mega charm and it didn't work.

How do I make it work?

You need to set the Pokémon Evolution to "By mega evolving" and choose the item it should hold.
